Learning to Achieve by Self-Reliance

 

Learning to Achieve by Self-Reliance (LASR) is a Disability Resource Connections program funded by the United Way of Greater Rochester. The LASR initiative provides people with disabilities who are not connected to a system of care and their caregivers support and direction to assist them in reaching their goals and managing life changes. Service navigators offer support to individuals for up to six months. Our program meets a specific set of quality practices and elements including:

 

• Community resource support and referrals for short-term needs

• Eligibility screening for, and connection to public assistance supports

• Person-centered planning to achieve self-sufficiency goals
