Job Title:                   Administrative Assistant – Mental Health Center

Location:                   Rochester Rehabilitation Center - Elmwood

 

Salary:                       $17.00 - $17.50/hr

Employment:         Full Time/12 Month, 40 hours per week, Monday through Friday

 

Purpose:

The Administrative Assistant provides administrative support services to the Manager of the Mental Health Center and therapist staff. Coordinates key office operations relating to the administrative functions of the mental health center.

 

Key Functions:

  • Provides support for departmental meetings including organizing, transcribing and distribution of minutes, as well as communication to staff regarding the meetings.

  • Creating and maintaining various reports and Quality Improvement Surveys as requested by the Manager and Coordinator.

  • Maintains and organizes departmental files and paperwork, including development of new forms and processes as assigned by the Program Manager and/or supervisor.

  • Provides support and coverage to the Registration Coordinator.

  • Coordinates translation services for client appointments.

  • Responsible for ordering and maintaining office supplies for the Mental Health Services Division

  • Participates in certain agency committees as representative for the Mental Health Services Division.

  • Adhere to Company policies and procedures to include being aware of the importance of Corporate Compliance Policies and the Code of Conduct.

Education, Experience, Skills and/or Licensing/Certification Required:

Associates Degree in secretarial science, office management or related area with at

least three years experience providing administrative support services, preferable in

a health care setting.  Accurate and efficient word processing skills.  Ability to work independently and exercise discretion in confidential matters.

Purpose:

To provide patient-centered, high quality care management services in a timely manner to clients with complex chronic conditions by navigating systems such as primary medical, specialty and behavioral health care, as well as social services.

 

Key Functions:

  • Provides comprehensive care management services including: outreach and engagement; assessment/reassessment; development and implementation of initial and revised individualized care plans; consult with primary care physician, and other care provider(s); identify crisis intervention plan.

  • Provides care coordination and health promotion including: collaborate with service providers and health plans to obtain medically necessary care, share crisis intervention(s) and emergency info; linkage to services to support care and treatment goals/plans; patient education; self-help recovery and self-management;

  • case reviews; advocate for services and assist with scheduling;

  • collaborate with providers to ensure service delivery; monitor, support,

  • accompany (as indicated) patient to/with scheduled appointments; crisis intervention.

  • Provides comprehensive transitional care including: follow up on notice of ER/hospital/residential/rehab setting admission and discharge; facilitate discharge planning with ER/hospital/residential/rehab setting staff to a safe location with care needs in place; notify service providers of admission/discharge and follow appointment needs; link with community supports; post discharge follow-up.

  • Provide member and family support including: care planning with patient/family; patient/family consultation and education around health care needs; patient/family meetings; patient/family referral to peer supports, support groups, social services and other programs as indicated; collaborate with community providers to support service utilization based on patient/family need.

  • Provide referral to community and social support services including: identifying and linking to community supports as needed; assure and assist with access to

  • medical, behavioral health, substance abuse and other community-based services as needed.

  • Timely and comprehensive completion of documentation including notes, assessments, tracking tools, etc.

Education, Experience, Skills and/or Licensing/Certification Required:

An Associate’s degree in Human Services field AND 4 years of experience OR Bachelor’s degree AND 2 years of experience in public or community mental health settings serving low income and culturally diverse populations. Computer literacy required. Valid New York State driver’s license with approved driving record and access to a reliable vehicle with proof of insurance for vehicle is required.

Job Title: Custodian

Location: Rochester Rehabilitation Center – University Ave

Salary:             $15.00/hr       

 

Employment:  This is a full time or part time position up to 40 hours per week            

Purpose:

To perform routine manual tasks in maintaining the cleanliness, sanitary conditions and appearance of assigned buildings. Follows the contract and quality control procedures.

 

Key Functions:

  • Cleans bathrooms, offices, hallways, and any other assigned areas by vacuuming, sweeping, mopping, scrubbing and polishing.

  • Empties and decontaminates trash containers.

  • Dust furniture and fixtures.

  • Dust mop and wet mop floors using industrial-size mops and/or machines

  • Washes blinds, windows and walls with prescribed solutions.

  • Reports needs for minor building or equipment repairs to supervisor.

  • Performs custodial related duties necessary to maintain cleanliness and sanitary conditions.

  • Performs many varied custodial tasks necessary for the upkeep of the buildings and grounds.

Job Title: Program Clerk

Purpose: The Administrative Assistant provides administrative support to the Director of Employment Connections as well as to the Director of One Source Solutions to prepare and review reports, maintain departmental files, initiate purchasing for the departments, coordinating all building, computer equipment and phone maintenance. This Assistant is also responsible for reception duties as the main contact for entrance to the building. Deals with a diverse group of external callers and visitors as well as internal contacts at all levels of the organization.

Key Functions:

1. Imparts agency philosophy of “maximizing each person’s ability to lead a full and active life providing the highest quality services.”
2. Maintains confidentiality of information relating to individuals and their families in accordance with agency policy.
3. Greets all visitors and employees entering the building, maintains visitor log and complies with the building security procedures.

Salary                     $22.50 – 25.00/hr

Employment       Full Time, 37.50 hours per week, Monday through Friday 8:00 am – 4:00 pm

 

Responsibilities include but are not limited to:

  • Guide a case load of 16-18 clients through the rehabilitation process by assisting each client in establishing and achieving his/her personalized life role goals.

  • Responsible for timely caseload documentation including concurrent development of clients’ individualized recovery plans (IRP), on-going progress notes, IRP reviews, and on-going assessment in accordance with NYSOMH regulations.

  • Participate in on-going development and implementation (teaching) of program curriculum based on clients’ barriers to goal attainment.

  • Coordinate outside referrals for clients as needed

  • Communicate with all members of the client’s treatment team to deliver a comprehensive coordination of care.

  • Responsible for the orientation and intake of new clients including comprehensive client assessments.

  • Demonstrated knowledge of a person-centered planning and ability to work as part of a fast-paced multi-disciplinary team.
